Sample Letter Request to Provide Reasonable Accommodations at Workplace
Below is a sample letter that an employee that needs and accommodation should provide to his employer as soon as such an accommodation is needed, even if you don't know what exact accommodation you need at that point:

Dear Supervisor / Director / HR Manager,
As you may know, I suffer from (name of disability/illness) / I recently suffered an injury to my back.. I would like to have an opportunity to discuss with you, or anyone else who is authorized to do this at the company, how my condition can be reasonable accommodated, as provided by ADA and FEHA (Fair Employment and Housing Act). Please contact me at your earliest convenience to discuss. I will be happy to provide the necessary medical documentation to support my request for accommodation.

Send the above request to more than one manager, if possible. Ideally, you should submit one copy to your immediate supervisor, and one copy to your human resources manager, charged with compliance with disability laws at workplace.
